Reduction Request Form - Print out this form and complete to request a reduction for a Vehicle and Traffic Offense. ** Note - this form is for use only with tickets issued by the Village of Tupper Lake Police Department. Mail the completed form to: Village of Tupper Lake Justice Court, P.O. Box 1290, 53 Park Street, Tupper Lake, NY 12986.

MV-104 Civilian Accident Reports - All drivers of a motor vehicle involved in a "reportable" accident must file an MV-104 report with DMV. This is a two page form, page two gives the instructions. The form is editable when opened and can be filled in and printed right from your screen.
