• MV-104 Civilian Accident Report - All drivers of a motor vehicle involved in a "reportable" accident must file an MV-104 report with DMV. This is a two page form, page two gives the instructions. The form is editable when opened and can be filled in and printed right from your screen.

• Street Light Report - Use this form to submit information regarding a street light being burned out, damaged, or any other problem with a telephone pole that you notice. All submissions will be forwarded to the Village of Tupper Lake Electric Department.
